2012 HS Art Exhibition Entry Form Submission 

Please remember to review exhibition guidelines before submitting any forms.

This year, an individual entry form is available as well as the package of 20 forms. You may use whichever format you are most comfortable with.

The individual form is for one tag entry at a time. Each tag is then submitted using the drop-box below. This form contains a one-click save and print button at the bottom and was created for those in mind who experienced errors when they were saving the large package.

The package form containing up to 20 tags in one file makes it easier for teachers who fill out the forms for all of their students. Use as many multiple-tag packages as necessary to cover all students submitting work. Leave blank what you don’t need. Teachers will submit packages using the drop-box below. 

To submit INDIVIDUAL entry tags:

  1. Open the entry form.
  2. Complete all fields in the tag.
  3. Click Once to Save and Print Tags.
  4. In the drop-box below, select the entry form
    by browsing to the saved copy on your
    desktop.
  5. Enter your school name as the description
    and your email address.
  6. Click the Upload button.

To submit PACKAGE of up to 20 entry tags:

  1. Open the entry form.
  2. Fill out as many electronic tags as necessary.
    Leave unused tags blank.
  3. Print the tags for attachment to each work.
  4. Save the completed package to your desktop.
  5. In the drop-box below, select the package by
    browsing to the saved copy on your desktop.
  6. Enter your school name as the
    description and your email address.
  7. Click the Upload button.
