Report a Concern
Students, faculty and staff can share concerns, comments or report incidents to the proper offices using the forms below. When you take a moment to complete a report, you're helping maintain a safe, respectful, and supportive campus community.
Report a concern:
Academic Misconduct
Use this form to report academic misconduct. This may include but is not limited to the following:
- Plagiarism
- Cheating
- Fabrication
- Inappropriate use of AI
- Facilitation
Behavioral Intervention Team
Use this form to report instances to KCC's Behavioral Intervention Team. This may include but is not limited to:
- Observation of unusual behavior
- Mental health concerns
- Threats to oneself or another
Bias Incidents/Title IX/Hazing
Use this form to report incidents of bias. This may include but is not limited to:
- Racial or gender discrimination
- sexual harassment/discrimination
- Hazing
- Domestic violence
Candid Comments
Use this form to provide suggestions, comments and concerns about KCC.

Criminal Conduct
If you have been the victim of a crime or have observed a crime in progress, and do not want to report it for fear of being identified, please fill out the form below.
Instructional Complaint/Grade Grievance
The instructional complaint process addresses concerns related to grades, the classroom or faculty conduct. Please attempt to resolve the concern directly with the faculty member before starting the formal resolution process. If the issue is not addressed, a formal process may be initiated.
Outcome Appeal Form
This form can be used to appeal an outcome of a decision made in reference to the KCC code of conduct, student instructional grievance, or program handbook violation (health careers or athletic).
Student Misconduct
Use this form to report general misconduct incidents to KCC's Student Conduct. This may include but is not limited to:
- Disruptive or inappropriate behavior
- Bullying
- Retaliation
- Student organization violations
Student Alerts
(for Faculty & Staff only)
Student alerts can be raised by faculty and staff on my.kcc.edu portal. Alerts go to an advisor or success coach to provide the student with additional support. This may include but is not limited to:
- Excessive absences
- Course grade falling below 75%
- Low test scores
- Tutoring required
